Summary

RMO No. 01-2007 amends the User Agreement Form (UAF) and certain provisions of RMO No. 25-2006 on accomplishing, signing and submitting the UAF. The UAF is required for granting Integrated Tax System (ITS), e-mail and internet access to revenue personnel.

Key points

  1. UAF filled out once. Revenue personnel accomplish the UAF only once. It covers all their later requests for email, internet and ITS account access.
  2. No new UAF on transfer. Personnel or others concerned who transfer to another office need not accomplish another UAF. Exceptions are circumstances determined by the Deputy Commissioner for Information Systems Group or her authorized representative.
  3. National Office requests. The UAF and Request for System Access Form (BF 0044) go to the Security Management Division-Information Planning and Quality Service (SMD-IPQS) for action. This includes ITS/server access, re-activation of a suspended account, password reset, and replacement of or additional job designation.
  4. Regional, RDO and LTS requests. The UAF and BF 0044 go to the respective Revenue Data Center (RDC) for validation/review. The RDC then endorses the request to SMD-IPQS for action.
