Key points

  1. Requisition. Blank CAPs are requisitioned from the Accountable Forms Division, which keeps a permanent record book. The Chief, Administrative Division of the Revenue Regional Office requisitions BIR Form No. 2330, and the Chief, Administrative Unit requisitions CAPs from the Administrative Divisions. Each requisition needs the Regional Director’s written approval and the requisition and issue slip signed by the Division Chief.
  2. Issuance. The authorized Revenue Officer prepares the CAP right after proof of payment of the advance VAT or Percentage Tax is presented. The Revenue District Officer approves and issues it the same day. A photocopy of the proof of payment is attached to the RDO file copy for post-verification and validation.
  3. Distribution. The original goes to the taxpayer, the duplicate is the RDO file copy, and the triplicate is for the authorized Revenue Officer.
