RMO No. 39-2014: Prescribes the use of the revised BIR Form No. 0044 (Request for System Access Form)

Key points

1. New items in the form. The revised form adds Item 8 (Email address), an Item 12 entry for Additional Server/Name: eTIS, and Item 16 (Requirements Submitted), which lists the Acceptable Use Policy and the Non-Disclosure Agreement.
2. Replaces earlier form. The revised and reformatted BIR Form No. 0044, dated November 2014, supersedes BIR Form No. 0044 dated July 2008.
